Root Cause Analysis and Incident Investigation - 3-days (TD00106)
In this 3-day course, you will learn the powerful tools and techniques that are part of the SOURCET (Seeking Out the Underlying Root Causes of Events) methodology. The SOURCE methodology is based on tools commonly used throughout industry for incident investigation. ABS Consulting's unique application of these common tools will guide your investigation, making each investigation efficient and effective.

Root Cause Analysis and Incident Investigation for Supervisors - 1½-days (TD00123)
This 1½-day version of our popular Root Cause Analysis course is designed for personnel performing investigations of simple incidents. The focus of the course is identifying causal factors, root causes, and recommendations for human performance problems and equipment issues.

Advanced Root Cause Analysis and Incident Investigation - 4½ days (TD00306)
This 4½ day course is an advanced version of our 3-day course. In addition to all the topics covered in our 3-day course, you will participate in additional workshops related to: causal factor charting; cause and effect trees; interviewing; physical data analysis plans; near miss definitions and reporting; trending; and metrics.

Maritime Root Cause Analysis and Incident Investigation - 2 days (TD00106-M)
This course is a special adaptation of our root cause analysis (RCA) and incident investigation techniques for the maritime industry. This course was developed in partnership with the American Bureau of Shipping (ABS) and uses their Guidance Notes on the Investigation of Marine Incidents as a reference. We have modified the terminology, examples, and workshops to address the specific issues associated with maritime applications.

Root Cause Analysis for Healthcare Organizations - 2 days (TD00406)
In this workshop-intensive course, you will learn to apply root cause analysis to sentinel events. Discover the powerful tools and techniques of the versatile SOURCET (Seeking Out the Underlying Root Causes of Events) methodology. Our unique application of these common tools will guide your analysis, making each investigation more efficient and effective. We teach effective interviewing skills along with methods for collecting other types of data. Participants each receive a copy of our Root Cause Analysis Handbook and Root Cause Map. Realistic medical examples and workshops are used extensively throughout the course to illustrate key points and to allow you to practice your new skills.
