Learn to use the SOURCE™ Methodology for Investigating Incidents
In this workshop-intensive course, you will learn:
Why and how to define near misses and how to institute a program to
learn all you can from all incidents
How to initiate an investigation and gather evidence How
and when to apply powerful techniques for causal factor and root cause analysis
(the heart of this course) for investigating process and nonprocess incidents How
to avoid future incidents by developing appropriate recommendations to
address root causes How to structure reports for optimum
effectiveness The fundamentals of data trending that
allow identification of systemic problem areas
You will learn the powerful tools and techniques that are part of the SOURCE™ (Seeking
Out the Underlying Root Causes of Events) Methodology. These techniques are
based on techniques commonly used throughout industry for incident investigation.
Our unique application of these common tools will guide your investigation,
making each investigation efficient and effective. We teach effective interviewing
skills along with methods for collecting other types of data. You will learn
to analyze data using causal factor charting, (event and condition charting)
fault tree analysis, and change analysis. The root cause determination techniques
include the "5-Whys" technique and ABS's "Root Cause Map™". Realistic
industry examples and workshops are used extensively throughout the course
to illustrate key points and to allow you to practice your new skills.
You will also learn how to use ABS's RootCause LEADER™ software and
FREE Excel spreadsheet to document your investigations and perform analysis

Besides learning incident investigation skills, you will learn how to fully
meet requirements of standards and regulations for investigations of incidents
with health, reliability, environment, and quality impacts. You will also get
an overview of how to structure an effective investigation program for your
company, including how to define near misses, how to train others to recognize
and report incidents, how to classify incidents for assigning the appropriate
investigator(s), and how to trend data. You will take back to your company
an example of a successful incident reporting and investigation program.
And, unlike many root cause approaches, your company may use the techniques
and tools taught in this course "license-free."
